Diane Jacobsen, MPH, CPHQ, Director, Institute for Healthcare Improvement, is currently directing the Learning and Innovation Community on Improving Flow Through Acute Care Settings and serves as faculty for IHI’s reducing mortality content. She is also IHI content lead and Improvement Advisor for the California Healthcare-Associated Infection Prevention Initiative (CHAIPI). Ms. Jacobsen has been involved with IHI since 2002 as a director for IHI’s Reducing Surgical Complications and Reducing Hospital Mortality Rates (HSMR), co-director of IHI’s Spread Initiative and faculty for the 100,000 and 5 million lives campaigns. She is an epidemiologist with more than 20 years of experience in quality improvement, risk management, and infection control in specialty, academic and community hospitals.
